We are partnering with Orland Park Dental Services to bring an onsite dental clinic to our district!
Consent forms are required for participation. OPDS sees students with no insurance, Medicaid insurance, and private insurance. Their services are completed at NO COST to families. Please see the consent form for more information.
Dental forms are due back to your student's school nurse by December 20th. The clinic will be hosted in January or February and the date of the clinic will be emailed out to families once it is scheduled.
Highland and Monroe Center families should complete the paper form sent home with them. Please contact your student's school nurse if you need another copy or access it below.
